The 1992 World Congress of Anesthesiologists was held in Buenos Aires, Argentina, from September 6-11. The Congress was attended by over 10,000 delegates from over 100 countries. One of the major themes of the Congress was the increasing role of anesthesia in the care of critically ill patients. This is due in part to the aging population and the increasing number of patients with chronic diseases. As a result, anesthesiologists are now playing a more active role in the management of patients in the intensive care unit (ICU).

This article provides a comprehensive overview of the latest developments in anesthesia and the lung, as presented at the 1992 World Congress of Anesthesiologists. Topics covered include the use of new anesthetic agents, the management of difficult airways, and the care of patients with pulmonary disease.

New Anesthetic Agents

Several new anesthetic agents were introduced at the Congress. These agents include:

* Desflurane: A volatile anesthetic agent that is similar to isoflurane but has a lower blood-gas partition coefficient. This makes it more rapidly eliminated from the body, which can be an advantage in certain clinical situations. * Sevoflurane: Another volatile anesthetic agent that is similar to desflurane but has a slightly higher blood-gas partition coefficient. These new anesthetic agents offer several advantages over traditional agents. They are more rapidly eliminated from the body, which can reduce the risk of side effects. They also have a lower incidence of side effects, such as nausea and vomiting.

Management of Difficult Airways

The management of difficult airways is a major challenge for anesthesiologists. Difficult airways can be caused by a variety of factors, such as obesity, trauma, and tumors.

At the Congress, several new techniques for the management of difficult airways were presented. These techniques include:

* The use of video laryngoscopes: Video laryngoscopes provide a clear view of the glottis, which can make it easier to intubate difficult airways. * The use of fiberoptic bronchoscopes: Fiberoptic bronchoscopes can be used to visualize the airway and pass a breathing tube through the vocal cords. * The use of laryngeal mask airways: Laryngeal mask airways are a type of supraglottic airway that can be used to provide ventilation in patients with difficult airways.

These new techniques have made it possible to manage difficult airways more safely and effectively.

Care of Patients with Pulmonary Disease

Patients with pulmonary disease are at increased risk of complications during anesthesia. This is because pulmonary disease can lead to hypoxemia, hypercarbia, and other respiratory problems.

At the Congress, several new strategies for the care of patients with pulmonary disease were presented. These strategies include:

* The use of preoperative pulmonary function testing: Preoperative pulmonary function testing can help to identify patients at risk of complications during anesthesia. * The use of prophylactic antibiotics: Prophylactic antibiotics can help to prevent the development of pneumonia and other respiratory infections in patients with pulmonary disease. * The use of noninvasive ventilation: Noninvasive ventilation can help to improve oxygenation and ventilation in patients with pulmonary disease.
